Insurance & Superbills
At this time, Ben Brodhead is not an in-network provider with any insurance companies and does not bill insurance directly.
Upon request, a superbill can be provided that includes the appropriate diagnostic and billing codes for your appointment. Patients may submit this documentation directly to their insurance provider to determine whether reimbursement is available under their plan.
Insurance coverage and reimbursement policies vary. Patients are encouraged to contact their insurance company regarding their specific benefits.
