Established Client Session: $ 80

*Please note: At this time Ben is not an in-network provider and is not billing insurance directly with any insurance companies. He is happy to provide you with a super bill, which lists all appropriate codes for your specific treatment. You can submit the super bill to your insurance provider in order to seek reimbursement for your paid-in-full appointment.
